MEDICAL ATTENTION
A counselor trained in CPR and First-Aid will be on duty to administer to any illness, and/or minor injuries. Upon application for camp you will be asked to download a medical waiver form. All campers must send or fax this completed form to Elevate Lacrosse prior to camp or you will not be permitted to participate in any activities. It is advised that you make and bring a copy of this form with you to registration to avoid any delays in play. Campers should notify the directors prior to the beginning of camp of any existing medical conditions that may require our assistance/attention.

MEALS
Day Campers will be required to bring their own packed lunch and snacks. It is also recommended that campers bring a water bottle filled at the beginning of each day.

CAMP STORE
There will be a camp store on site where we will be selling the hottest in lacrosse clothing, equipment, and accessories. This store will operate at check-in/checkout and at the opening and closing of each day.

RULES
All Campers are expected to show utmost respect for staff, other campers, and facilities. Elevate Lacrosse strictly prohibits the use and/or possession of drugs, alcohol, and tobacco products. Any camper found in violation of these rules will be immediately dismissed from camp without refund of fees paid.
